Job summary

Centra Health is seeking a Resource Team RN to manage individualized patient care in Virginia, collaborating with physicians and multidisciplinary teams to implement and evaluate the care plan. The RN will handle full patient assignments, plan care, and ensure safety and quality in accordance with Virginia Board of Nursing standards.

The role requires a nursing diploma or ADN, with BSN preferred, and active Virginia licensure.

Qualifications

• Current RN license in the Commonwealth of Virginia or compact license to practice in Virginia.
• Basic Life Support (BLS) required.
• ACLS requirements may be waived for 100% remote employees—note: not applicable if on-site.
• BSN preferred for advanced practice and leadership roles.

Responsibilities

• Conducts an individualized patient assessment and prioritizes data collection per policies and protocols.
• Develops, modifies, and integrates a patient care plan with multidisciplinary input.
• Performs treatments and nursing interventions as ordered, and responds to urgent changes in patient condition.
• Evaluates patient responses to care and updates the plan accordingly.
• Educates patients and families, ensuring understanding and consent for care.
• Collaborates with the care team to ensure safe, effective, and culturally sensitive care.
