Overview

Job Title: Resource and Referral Specialist

Our mission revolves around providing essential services such as education, referral to substance use disorder (SUD) programs, support for homeless individuals, cultural education, and drug and alcohol prevention. We strive to empower individuals and families within the community, fostering resilience and wellness.

Responsibilities
  • Resource Navigation: Conduct comprehensive assessments of individuals’ needs and circumstances and provide personalized assistance and guidance in accessing relevant resources and services.
  • Referral Coordination: Collaborate with community partners, service providers, and agencies to facilitate referrals for clients to appropriate programs and resources, ensuring continuity of care and support.
  • Case Management: Maintain detailed records of client interactions, referrals made, and follow-up activities, and provide ongoing support and advocacy to address barriers and challenges.
  • Information and Education: Provide information and education to clients about available resources, eligibility criteria, application processes, and rights and responsibilities, empowering them to make informed decisions.
  • Crisis Intervention: Provide crisis intervention and support to individuals experiencing urgent or emergency situations, connecting them with appropriate services and resources.
  • Cultural Competency: Demonstrate cultural humility and sensitivity in all interactions, respecting and honoring the traditions, values, and perspectives of the Native American community.
  • Collaboration: Collaborate closely with other members of the Native C.O.R.E. team, including outreach coordinators, program staff, and community partners, to ensure coordinated and effective service delivery.
  • Documentation and Reporting: Maintain accurate and up-to-date documentation of client interactions, services provided, and outcomes achieved, and prepare regular reports as required.
Suggested Qualifications
  • Bachelor’s degree in social work, psychology, human services, or a related field.
  • Minimum of 1 year of experience in case management, resource coordination, or social services, preferably in a nonprofit or community-based organization.
  • Knowledge of and experience working with Native American communities, with a demonstrated commitment to cultural competency and understanding.
  • Strong interpersonal and communication skills, with the ability to build rapport, establish trust, and communicate effectively with diverse populations.
  • Ability to work independently and as part of a team, with a proactive and solution-focused approach to service delivery.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple deadlines.
  • Proficiency in Microsoft Office Suite and experience with database management preferred.
  • Valid driver’s license and willingness to travel locally as needed.
