Referral and Patient Care Coordinator – MD157

Native American Rehabilitation Association of the Northwest, Inc. (NARA) – Part‑time

Location: This position will float between two clinics: Indian Health Clinic, 703 NE Hancock St., Portland, OR 97212 and Wellness Center, 12360 E Burnside St., Portland, OR 97233.

Schedule: Monday through Friday, 8:30 am–5:00 pm (in‑person).

Wage Range: $24.15–$27.30 hourly, Non‑Exempt.

Position Overview

The Referral and Patient Care Coordinator provides care coordination to ensure appropriate follow‑up of processed referrals, patient navigation, and timely provider review of medical documentation.

Key Responsibilities

• Provide patient navigation regarding referrals, including making appointments, arranging transportation, coordination of interpreter services, and modifying referrals based on patient preferences.
• Address client inquiries, questions, and concerns regarding enrollment, claims, benefit interpretation, and referrals/authorizations for healthcare or other resources.
• Receive, respond to, and direct client, provider, and insurance vendor phone calls; provide information and escalation when appropriate.
• Follow up with referrals to determine completion and ensure documentation from completed referrals is obtained and present in the EHR for provider review.
• Understand insurance coverage benefits and limitations and resolve pre‑certification, insurance, and case‑related concerns prior to appointments.
• Document client information and grant/insurance billing accurately in the electronic health record.
• Track all referrals made within medical and dental services.
