Qualifications

• Bachelor’s in a human services field preferred.
• At least 1 year of experience working with individuals with mental illness.
• 2+ years in case management, patient care, or social services preferred, especially inpatient or acute settings.
• Proficient in EPIC and standard office technology.
• Background check and drug screening required.

Responsibilities

• Coordinate intake through discharge for patients and families.
• Initiate treatment planning and coordinate interdisciplinary care to meet clinical and emotional needs.
• Support collaboration within a multidisciplinary inpatient behavioral-health team.
• Maintain clear communication to ensure care is well-coordinated throughout the treatment process.
