Schedule: Monday – Friday plus on‑call and weekend requirements.

Location: Serving the Greater Twin Cities Metro Area (21‑County Service Area).

What You’ll Do

• Admissions: Complete hospice eligibility assessments, consents/education, med reconciliation, immediate symptom plan, and safe‑start services.
• Case Management (as assigned): Own a patient panel—plan of care, symptom management, caregiver coaching, IDT coordination, timely documentation.
• Visit Nurse/Crisis Support: Provide PRN, routine, and urgent visits; execute standing orders; elevate and collaborate with the MD/NP swiftly.
• Interdisciplinary Collaboration: Partner with social work, chaplaincy, aides, and therapy to support patient and family goals.
• Documentation & Compliance: Chart accurately in the EMR; follow Medicare CoPs and agency policies.
