Job Title: Resource Nurse (Hospice) — Admissions, Case Management, Visit Nurse

Pay: $90,000 – $130,000 DOE

Benefits: Full Benefits Package

Schedule: Monday – Friday plus on‑call and weekend requirements.

Location: Serving the Greater Twin Cities Metro Area (21‑County Service Area)

What You’ll Do

• Admissions: Complete hospice eligibility assessments, consents/education, med reconciliation, immediate symptom plan, and safe‑start services.
• Case Management (as assigned): Own a patient panel—plan of care, symptom management, caregiver coaching, IDT coordination, timely documentation.
• Visit Nurse/Crisis Support: Provide PRN, routine, and urgent visits; execute standing orders; accelerate and collaborate with the MD/NP swiftly.
• Interdisciplinary Collaboration: Partner with social work, chaplaincy, aides, and therapy to support patient and family goals.
• Documentation & Compliance: Chart accurately in the EMR; follow Medicare CoPs and agency policies.

What You’ll Bring

• Required hospice background from one of the following: prior hospice agency experience or high‑acuity settings such as ICU, palliative care, or oncology.
• Strength in admissions interviewing, symptom triage, and family education.
• Organized, decisive, and comfortable driving across the service area (reliable vehicle/insurance).
