Open Sky Community Services is seeking a Residential Counselor to support individuals in residential and community-based settings, helping with daily living activities and goal attainment while ensuring a safe, supportive environment.

You will advocate for people served, promote community engagement, and collaborate with a multidisciplinary team in Central Massachusetts. A high school diploma and valid driver’s license are required, with reliable transportation for community activities.

Qualificações

• High school diploma or equivalent required.
• Valid driver’s license and acceptable driving record required.
• Reliability, willingness to learn, and openness to new opportunities.

Responsabilidades

• Assist in developing individualized, person-centered service and support plans and play an active role in their implementation.
• Provide support with daily living activities, skill development, and personal goal achievement.
• Assist individuals with accessing transportation and community resources as needed.
• Implement activities that promote community inclusion, relationship building, personal growth, and the development of valued social roles.
• Maintain a safe, clean, and supportive environment that promotes health, safety, and well-being of individuals served.
• Review, complete, and maintain all required documentation in an accurate and timely manner.
