Job summary

Momentum seeks a Residential Services Specialist for Crossroads II in San Jose, CA. The role involves providing milieu and mental health services, case management, and coordination with other programs to support residents.

You will develop treatment plans, facilitate psych and group activities, manage documentation, transportation, and after-hours coverage as part of a dedicated team.

Qualifications

• High School diploma, GED, or equivalent education plus five years’ related experience.
• Two years of experience in residential mental health or crisis programs preferred.
• Ability to work independently and in a team; strong English communication.

Responsibilities

• Provide milieu and/or mental health, rehabilitation and case management services in residence.
• Develop and implement individual service and treatment plans.
• Coordinate care with other programs and contact case managers.
• Enter and maintain up-to-date clinical documentation for each client.
• Provide transportation in personal or agency vehicles when needed.
• Offer on-call and floor coverage; support after-hours as needed.
