Job summary

DePaul is seeking a compassionate mental health professional to join Kensington Square, a CR-SRO program in Buffalo, NY. The Residential Services Coordinator 3 will support residents through individualized planning, recovery-oriented services, and advocacy to enhance autonomy and quality of life.

Qualifications

• Bachelor’s degree in Human Services with at least 1 year of experience.
• Associates degree in Human Services with at least 3 years of related experience.
• High School diploma / GED with at least 5 years of related experience.
• Clean/valid New York State driving record required, age 18+.

Responsibilities

• Tour prospective residents with Management for admissions input.
• Develop comprehensive, person-centered service plans and coordinate related services.
• Conduct quarterly service plan reviews with residents and support teams.
• Provide recovery-oriented supports and skills training per OMH standards.
• Maintain accurate resident files and progress notes; update plans accordingly.
• Schedule and arrange annual physicals, exams, and medical appointments with transportation.
• Collaborate with Health Home Care Manager and community providers for continuity of care.
• Monitor resident mental health and well-being in collaboration with clinical providers.
• Assist with daily living skills, meal preparation, laundry, and shopping for residents.
• Support residents in building healthy family and community connections and plan recreational activities.
• Assist Medication Coordinators with supervising medications and dining room duties when needed.

Education

Bachelor’s degree in Human Services
