Job summary

DePaul is hiring a Residential Services Coordinator in Rochester, NY. This full-time role focuses on developing and coordinating the Residential Service Plan to promote resident autonomy, recovery, and quality of life.

Responsibilities include teaching and monitoring recovery-oriented supports, planning admissions and discharges, coordinating with counselors, and ensuring medical and dental appointments are arranged with transportation when needed.

Qualifications

• Bachelor’s or Associate degree in a human services field with related experience.
• Clean NYS driving record required.
• Must be at least 18 years old.

Responsibilities

• Develops Residential Service Plan with resident and support network.
• Provide teaching, monitoring and recovery supports per OMH guidelines.
• Ensure comprehensive person-centered planning for admission, services, and discharge.
• Participate in monthly reviews with Counselor and Resident.
• Conduct quarterly reviews of the service plan and revisions as needed.
• Guide Counselors to monitor and report resident progress and attendance.
• Coordinate scheduling of medical and eye/dental appointments; arrange transportation.
• Collaborate with community providers to ensure continuity of care.
• Liaise with clinical providers to maintain resident mental health engagement.
• Assist residents with purchasing clothes and other necessities.
• Encourage healthy family/community connections; maintain confidentiality.
• Ensure safety and perform other program duties.
