Job summary

Project HOME is seeking a Residential Service Coordinator II to support the Co-Ed Lower Level Community & Hospital Transitions program at Sacred Heart Recovery Residence in Philadelphia. You will work with residents with complex medical, behavioral health, and substance use needs to coordinate recovery plans and connect them to essential services.

Qualifications

• Experience with homelessness and behavioral health is preferred.
• Desire to work with adults with complex medical needs.
• Availability for varied shifts and on-call duties.

Responsibilities

• Coordinate support and resources to assist residents' transition from homelessness.
• Design recovery plans with short- and long-term goals and respond to crises.
• Provide access to recovery, housing, benefits, employment, education, and medical referrals.
• Lead group activities and act as point person when needed.
• Assist with lease requirements and housing applications; gather documentation.
• Maintain resident records and use electronic records systems; participate in on-call duties.
