Job summary

Project HOME is seeking a Residential Service Coordinator Level 1 for the Male Recovery Long-Term Floor in Philadelphia. The role focuses on empowering residents toward stability, wellness, and self-determination through person-centered care, peer support, and community connections.

The position involves on-site engagement, crisis intervention, and helping residents achieve housing and employment goals while fostering an LGBTQ-affirming environment.

Qualifications

• CPS or Certified Recovery Specialist or two years experience with target population.
• Experience with homelessness or behavioral health challenges.
• Availability for evening, weekend, and on-call duties.

Responsibilities

• Provide initial orientation to new residents transitioning from homelessness and help design recovery plans with short and long-term goals.
• Assist residents in crises by assessing situations and coordinating referrals for recovery, housing, benefits, employment, education, medical care, and supportive services.
• Collaborate with residents to identify strengths, plan group activities, and build life skills through community engagement.
• Maintain resident records in an electronic system, including case notes, goal plans, and assessments; assist with housing applications and meetings.
• Alternate on-call duties with other staff for emergencies.
