Job summary

Hartford HealthCare’s Rushford Center is seeking a Case Manager Non-Clinician to work with individuals facing mental health or substance use disorders. You will assess needs, coordinate services within an integrated recovery framework, and advocate for clients to achieve wellness and independence.

The role requires an Associate’s degree (Bachelor’s preferred), at least 2 years of relevant experience, and a CT driver’s license.

Qualifications

• Associates degree required; Bachelor's degree preferred.
• Minimum 2 years of experience with individuals with mental health and/or substance use disorders.
• Valid Connecticut driver's license required.
• Public Service License qualification preferred.

Responsibilities

• Assess client needs and develop treatment goals with the client and Clinical Team.
• Present case reviews to Treatment Team to form a multidisciplinary Treatment Plan.
• Provide crisis intervention when needed and support clients with their treatment plans.
• Offer group interventions and involve family as appropriate in the treatment process.
• Orient clients to level of care, rights, Rushford policies and expected outcomes.
• Coordinate discharge planning and complete records per Rushford policy.
• Reference community resources and referrals to support Treatment and Continuing Care Plans.

Education
