Job summary

Lakeview Health Services, Inc. seeks a Residential Support Specialist for Woodland Commons Macedon, NY. You will collaborate with residents, teach skills for independent living, and use trauma-informed, person-centered practices.

Responsibilities include developing daily schedules, using motivational interviewing, facilitating activities, and maintaining progress updates. A NYS driver's license is required; HS diploma with experience or degree preferred.

Qualifications

• High School Diploma with two years of relevant experience or an associate degree in human services or related field or bachelor’s degree (preferred).
• Valid NYS Driver’s License (driving is an essential function).
• May be required to lift (minimum of 35 lbs.).

Responsibilities

• Collaborates, coaches, and aids individuals with mental illnesses in a residential setting.
• Teach and support skills for independent living using recovery-focused, person-centered practices.
• Develop person-centered service plans for individuals on your caseload.
• Assist residents in monitoring progress toward goals and provide updates.
• Document interactions related to service plan goals.
