Job summary

Qualifications

• Designated Coordinator: degree in human services/education/health plus required experience.
• Associate degree with required years of experience.
• Diploma in related field with required years of experience.
• Education hours and four years direct care experience under supervision.

Responsibilities

• Participate in individual planning with each person served.
• Collaborate with the care team and facilitate team meetings.
• Promote hobbies and recreational activities for residents.
• Assist with meal planning, shopping, and cooking.
• Schedule and attend medical appointments and provide medication administration.
• Educate and problem-solve regarding safety, finances, and self-advocacy.
• Supervise Direct Support Professionals.
• Train personnel on health and support needs to promote independence.
• Manage and maintain staffing schedules to meet needs.
• Collaborate with program manager on staff training and coaching.
