Qualifications

• High school diploma or GED; 2+ years in human services preferred.
• Bachelor’s degree preferred in a related field.
• CT driver’s license with a good driving record.

Responsibilities

• Create a nurturing, positive learning environment for clients.
• Develop strategies to help cope with daily living and independence.
• Lift and transfer clients (70-75 lbs) and work inside/outside the home.
• Apply approved safety and protective hold techniques when needed.
• Follow care plans, document progress, and report barriers to the team.
• Monitor medical/behavioral changes; assist with appointments and follow-ups.
• Help plan meals, budgets, and ensure dietary plans are followed.
• Maintain cleanliness and safety of the home, vehicles, and related spaces.
• Communicate professionally with participants, families, and staff.
• Participate in required training and maintain documentation and compliance.
