Job summary

Qualifications

• Bachelor’s Degree required; two years’ IDD experience.
• Supervisory experience required.
• Ability to document observations and communicate clearly.
• Must hold a valid North Carolina driver’s license.

Responsibilities

• Lead the habilitation team for each consumer.
• Plan and write consumer programs and annual habilitation plans.
• Submit Bi & annual service authorization requests.
• Ensure State and Federal regulations are followed at all times.
• Serve as liaison with school systems and community agencies.
• Plan, write and implement plans of correction from a State survey.
• Intake all new consumers and document procedures.
• Review Resident Incident Reports and identify patterns.
• Coordinate communication among facilities and families.
• Provide clinical supervision to paraprofessionals and supervise home managers.
