Summary
Maximize residents' success, satisfaction, and ownership in their living and working environment through effective skills training and relationship building. Maintain a safe, clean, and therapeutic environment within the residential program. Provide client-driven services that ensure quality and excellence in client care and support client recovery.
Responsibilities
- Available to cover shifts as assigned at any residential program, including traveling between programs.
- Facilitate ADL skill-building for residential living and community integration, supervising and teaching clients daily routines, medication management, budgeting, self-preservation skills, transportation, etc.
- Participate in developing and implementing client treatment plans with a multidisciplinary team, engaging clients in planning and evaluation.
- Coordinate and monitor treatment services for all assigned clients, including contact as prescribed, care coordination, and monitoring care effectiveness.
- Provide shift coverage, household maintenance, conflict resolution, crisis management, and symptom management.
- Attend supervision and team meetings as required.
- Maintain knowledge of agency policies and procedures.
- Work flexible hours including first and second shifts, holidays, and weekends.
- Ensure safety and security of office and residential areas.
- Schedule and facilitate treatment plan meetings, utilizing assessment data and involving clients and families.
- Screen for substance abuse, provide assessments, and refer as needed.
- Perform other duties as assigned.
- Remain on shift until appropriately relieved, possibly working consecutive shifts.
Qualifications
- High school diploma or GED.
- Flexibility in work schedule, including weekends and holidays.
- Ability to perform household chores and lift up to 35 pounds.
- Effective communication skills face-to-face and by phone.
- CPR and first aid certification, and ability to monitor vital signs.
- Valid driver’s license and ability to transport clients.
