Odyssey House is seeking a dedicated Case Manager for OASAS Residential programs in East Harlem, the Bronx and Ward's Island, NY. You will provide individual counseling and comprehensive case management to clients addressing medical, psychiatric, housing and vocational needs, while collaborating with the clinical team to promote recovery and a drug-free lifestyle.

Formación

• High School Diploma or GED required.
• Two years' experience as a case manager in a residential setting preferred.
• CASAC-T preferred.
• Must be able to work a flexible schedule.

Responsabilidades

• Provide individual counseling and case management addressing medical, psychiatric, housing and vocational concerns.
• Formulate and monitor treatment plans with regulatory requirements and clinical indicators.
• Maintain up-to-date client records including treatment plans and progress notes per guidelines.
• Conduct education/prevention groups on chemical dependency and related issues.
• Provide crisis intervention services.
• Responsible for ongoing client discharge planning.
• Assist with room inspections and ADL support as needed.
• Facilitate house and community meetings and liaise with referral agencies.
