Job summary

Odyssey House in New York City seeks a Case Manager for the Haven program. You will provide counseling and intervention services to tenants dealing with substance use, HIV/AIDS, and mental health issues.

You will manage a caseload of 20–25 tenants and coordinate home visits with the care team. The role requires at least a High School Diploma or related experience, with a Bachelor's in Human Service preferred.

Qualifications

• High School Diploma required or two years Odyssey House experience and a HS diploma, or a Bachelor's in Human Service.

Responsibilities

• Provide substance abuse counseling and intervention services.
• Stabilize and promote self-sufficiency and healthy living for tenants.
• Manage a caseload of 20–25 tenants, including home visits.
• Attend case conferences with HRA/HASA and provide evaluations.
• Maintain up-to-date tenant charts, case notes, and service plans.
• Other duties as assigned by supervision.
