Job summary

Northern Rivers Family of Services seeks a dedicated Case Manager to support clients in the Residential System and collaborate with clinicians. This onsite role involves home visits, transition planning, and coordination of Independent Living Services as part of an interdisciplinary team.

The position requires strong case documentation, liaison with schools, and primary contact responsibilities with families and DSS. Travel to client locations is expected.

Qualifications

• Two+ years in social services or case management.
• Bachelor’s degree preferred for advanced roles.
• Experience with care systems is helpful.

Responsibilities

• Arrange and attend home visits.
• Prepare transition plans.
• Coordinate Independent Living Services.
• Be the primary contact between Schools and Residential.
• Coach family visits.
• Complete assessments such as FASP and SPR.
• Input case notes into the Connections database.
• Provide client transportation.
• Maintain contact with DSS and families.
• Attend dorm and treatment review meetings.
