Job summary

Centerstone is seeking an intensive case manager to join our behavioral health team in Bloomington, IN. You will assess client needs, develop individualized care plans, coordinate wrap‑around services, and connect clients with community resources while ensuring program compliance.

Work with a multidisciplinary team, provide transportation support when needed, and document outreach efforts to support positive outcomes for individuals and families served.

Qualifications

  • HS Diploma or equivalent and two years of relevant experience.
  • Associate's degree preferred.

  • Communicates treatment plans to the client and the family and monitors progress.

JOB DESCRIPTION:
SUMMARY OF POSITION

Provides intensive case management services designed to support individuals and families. Works with the service team to conduct assessments and develop/monitor Individual Care Plans. Provides assistance to clients in identifying/retaining resources as needed, identifies family service needs, refers clients to services as necessary, and provides wrap-around support and services. This includes performing direct client and/or community services as necessary to meet program service obligations, assuring compliance with contracts, grants, standards, regulations, and laws.

ESSENTIAL DUTIES & RESPONSIBILITIES
  • Assesses treatment needs and develops patient care plans accordingly. Work closely with other team members to determine the needs of each participant in order to determine the level of services needed.
  • Completes assessments and works with clients to develop and implement the Individual Service Plan. Communicates treatment plans to the client and the family. Monitors and tracks participant's accomplishments of goals during active and follow up phases and updates the plan as necessary.
  • Provides outreach, advocacy, support and follow-up to assigned clients. Documents all outreach attempts and ensures all services/outreach indicated in the plan are provided in accordance with identified frequency.
  • Assists client in building a support team. Integrates natural community supports/resources into the planning and service delivery process.
  • Develops and maintains relationships with local service and community resource agencies that can benefit clients.
  • Provides transportation assistance as needed to help participants achieve and maintain case management goals.

  • Effectively communicate via written, verbal, in person and virtual methods.

QUALIFICATIONS
Education Level

HS Diploma or equivalent and two years or relevant experience. Associate's degree preferred.

Two years of relevant experience with a HS Diploma.
