Job summary

Centerstone is a national nonprofit behavioral health system seeking a Case Manager to coordinate services, provide coaching, and connect patients with wellness resources. A bachelor's degree in counseling, psychology, social work, or related field is required, with a focus on whole-person care across clinic, home, and community settings.

  • Develop, implement, evaluate, and update treatment plans with input from client and family.
  • Enter written documentation into Electronic Medical Record for all services rendered.
  • Connect with clients face to face in the office and community, telehealth, and by phone.
  • Attend weekly staffing and team meetings and complete assessments.
  • Assist clients with developing natural support systems.
  • Complete all required agency trainings to maintain 100% compliance.

JOB DESCRIPTION

SUMMARY OF POSITION Jump start your career in the mental health field as a Case Manager. You'll coordinate services and care for patients – providing coaching, strategies and coordination of health and wellness resources so they can live their lives more fully. Through your work, you will teach people the importance of the connection between physical and mental health. If you have a bachelor's degree in counseling, psychology, social work or related field – and you're passionate about making a difference in the lives of people coping with mental health issues, we need you! You will become a trusted resource for your patients, offering connection and education about how recovery works best when we address the whole person. You will enjoy the flexibility of working in a variety of settings – the clinic, patient homes and in the community. ESSENTIAL DUTIES & RESPONSIBILITIES

  • Provides supportive and educational case management services to develop daily living and coping skills.
  • Responsible for developing, implementing, evaluating, and updating treatment plans with input from the client and family.
  • Enters written documentation into Electronic Medical Record in a timely manner for all services rendered.
  • Connect with clients face to face in the office and community, telehealth, and by phone to address needs
  • Demonstrate knowledge and understanding of ethical standards by evidence of practice with patients and community partners.
  • Attend weekly staffing and team meetings and completes assessments
  • Assist clients with developing natural support systems.
  • Completes all required agency trainings to maintain 100% compliance.
  • Maintains departmental productivity expectations.
  • Attend and participate in clinical supervision.
KNOWLEDGE, SKILLS & ABILITIES

Education Level: Bachelor's Degree in psychology, social work or related field required. Years of Experience: Experience with Electronic Health Record preferred. Certification/Licensure: Valid driver's license in good standing required.
