Location: 2936 Main Street, Bridgeport (10518)

Shift: 40 hours per week, Monday through Friday, 8:30 am - 5:00 pm; may require some weekends and holidays; on-call duties once a month.

Job Summary

Investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.

Responsibilities
  1. Interview patients, assess priorities, and document case activity.
  2. Consult with team managers, agencies, and relatives to ensure compliance with treatment plans.
  3. Assist clients/families with community resources.
  4. Develop and monitor individualized Residential Program Plans.
  5. Respond to residential incidents on a 24-hour on-call basis.
  6. Assist residents with personal care, including coaching and teaching daily living activities.
  7. Provide transportation for appointments and activities.
  8. Complete assessments and progress notes, coordinating with family and community providers.
  9. Perform other duties as assigned.
Qualifications

Education: Bachelor's Degree required; Human Services, Social Work, Psychology, or related preferred. Equivalent education and experience considered.

Work Experience: Experience with community-based programs for psychiatric and addiction disorders; experience with the homeless is helpful.
