• 3 years recent experience with elder population or chronic disability for Program Assistant at Day Health.
• Valid Driver’s License with insured vehicle required.

Responsibilities

• Assist the population living with chronic behavioral health disorders.
• Help with daily living activities and medication management.
• Provide outreach services as needed.
