• HS diploma and 1+ year of field related experience.
• Health care related field toward AS or BA preferred.
• Excellent communication and writing skills.
• Ability to promote a teamwork environment and serve as a role model.

Responsibilities

• Advocate for client caseload.
• Develop, implement, and monitor treatment plans.
• Oversee medication management.
• Fiscal management of funds.
• Serve as primary liaison for service providers, family, and guardians.
• Utilize an electronic health record system (Avatar) for client notes.
