Aspire Health and Community Services is seeking a Residential Rehab Coordinator to support adults in our residential program. You will help clients reach ISPs, manage medications, and coordinate services in a supportive, therapeutic environment.

Qualifications

• HS diploma and 1+ year of field related experience.
• Strong written and verbal communication.
• Ability to use an electronic health record system.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Advocate for client caseload and coordinate care.
• Develop, implement, and monitor treatment plans.
• Oversee medication management and fiscal funds.
• Serve as liaison with providers, families, and guardians.
• Utilize Avatar for client notes and reporting.
• Support residents’ daily living skills and ISP goals.
