Job summary

Responsibilities

• Engage and support members in residence or apartment to assist with upkeep and well-being.
• Work with FH clinical staff, case management, vocational staff, and social practitioners as part of an integrated care team.
• Establish and update goals and record progress of members; conduct crisis de-escalation when needed.
• Maintain daily log entries regarding complaints and incidents.
• Attend staff meetings and trainings; perform other duties as assigned.
