Job summary

Maryland Gardens Post Acute is seeking a Social Services Coordinator to assist in planning, developing, organizing, implementing, evaluating, and directing the facility's social service programs in line with federal, state, and local standards.

The role involves collaborating with administration, medical and nursing staff, and other departments to ensure residents receive comprehensive social care and resources.

Qualifications

• Must possess, as a minimum, a high school diploma or its equivalent; Associate or Bachelor's Degree preferred but not required.
• Prior health-care experience preferred.
• Ability to read, analyze, and interpret general business materials and regulations.
• Ability to write reports, business correspondence, and procedure manuals.
• Ability to present information clearly to managers and staff.
• Mathematical skills with fractions, percentages, and ratios.

Responsibilities

• Assist the Social Service Director/Consultant in planning, developing, organizing, and directing social service programs.
• Meet with administration and staff to plan social services.
• Develop, administer, and coordinate department policies and procedures.
• Review policies annually and suggest changes.
• Identify residents' medically related social and emotional needs and plan accordingly.
• Coordinate social service activities with other departments.
