Qualifikationen

• Graduate of an accredited School of Nursing.
• Current Massachusetts LPN or RN license required.
• BSN preferred.
• Minimum of three years of nursing experience required.
• Experience in senior living, geriatrics, home care, care management, wellness nursing, or community-based services preferred.

Aufgaben

• Conduct clinical assessments for prospective Independent Living residents to identify health needs and considerations for successful community living.
• Provide ongoing wellness assessments for residents in the Medication Management Program and support medication reconciliation and communication with providers and pharmacies.
• Perform wellness follow-up visits as needed and document clinical information accurately for billing and care coordination.
• Lead and participate in clinical staff education, in-service trainings, and quality improvement initiatives across Health Services and Glenmeadow at Home.
