Job summary

Glenmeadow seeks a Community Wellness Nurse & Clinical Educator to support health, safety, independence and well-being of residents, focusing on Independent Living. You will oversee Medication Management, wellness visits, and clinical triage, plus lead staff education and quality initiatives across Health Services.

Massachusetts LPN/RN license required; BSN preferred, with a minimum of three years in geriatrics or related fields. This full-time role is based in Longmeadow, MA.

Qualifications

• Graduate of an accredited School of Nursing.
• Current Massachusetts LPN or RN license required.
• BSN preferred.
• Minimum of three years of nursing experience required.
• Experience in senior living, geriatrics, home care, care management, wellness nursing, or community-based services preferred.
• Experience providing resident education, staff training, clinical in-services, or wellness programming preferred.

Responsibilities

• Conduct clinical assessments for prospective Independent Living residents to identify health needs, potential risks, and considerations for successful community living.
• Provide ongoing wellness assessments, monitor changes in health status, and support medication reconciliation and communication with providers and pharmacies.
• Lead clinical staff education, in-service training, competency development, and quality improvement initiatives across Health Services.
• Coordinate care with Care Management, Glenmeadow at Home, Assisted Living, and other departments.
• Maintain accurate clinical documentation in accordance with community standards and regulatory requirements.
