Job summary

Nazareth Living Center is seeking a Wellness Assistant to enhance residents' quality of life through social, spiritual, therapeutic, and other wellness activities within the community.

The role involves developing wellness programming, facilitating daily events, collecting data for assessments, and supporting the plan of care based on residents’ needs. The position emphasizes compassionate service and collaboration with the care team.

Qualifications

• High School Diploma or equivalent.
• Experience in long term care.
• BLS Certification.
• First Aid Certification.
• Program leadership qualifications per federal regulations §483.15(f)(2).

Responsibilities

• Assists in developing the community’s wellness programming.
• Facilitates daily events and programming to meet residents’ needs.
• Performs data collection to assist with residents’ assessment processes.
• Assists in implementing the plan of care based on residents’ needs.
