Job summary

Paylocity is seeking a SNF Transporter in California to safely transport residents within the facility and to off-site appointments, using proper transfer techniques and infection control.

You will coordinate with nursing staff to ensure resident safety and comfort, maintain transport logs, and assist with loading/unloading equipment as needed. Prior CNA experience is a plus, and HIPAA compliance is required.

Qualifications

• High school diploma or equivalent.
• Valid California driver's license with a clean DMV record.
• Ability to safely operate facility transport vehicles.
• Basic understanding of resident safety and proper body mechanics.
• Strong communication skills and ability to work as part of a care team.

Responsibilities

• Safely transport residents via wheelchair, gurney, or facility vehicle to and from appointments, procedures, and activities.
• Assist residents with transfers in and out of vehicles, wheelchairs, and beds using proper body mechanics and facility lift/transfer protocols.
• Verify resident identification and transport orders prior to each trip.
• Communicate with nursing staff regarding resident status, special needs, or precautions before transport.
• Ensure resident safety restraints/seatbelts are properly used during all vehicle transport.
• Maintain accurate transport logs, including departure/return times, destination, and any incidents.
• Report any change in resident condition observed during transport to nursing staff immediately.
• Maintain cleanliness and proper functioning of transport vehicles and equipment.
• Assist with loading and unloading medical equipment as needed (oxygen tanks, wheelchairs, etc.).
• Follow all infection control precautions during transport.
• Maintain resident confidentiality in accordance with HIPAA requirements.
• Perform other related duties as assigned.
