Job summary

Talentify is seeking a Transportation Aide to coordinate and deliver restorative nursing care while transporting residents to activities, appointments, and off-site services. The role supports the nursing department and rehabilitation team to keep patients physically capable and emotionally supported.

Candidates must be a state-registered CNA with an active Illinois driver's license and may require Class C or Class E licensing depending on vehicle size.

Qualifications

• Must be a state-registered CNA with active Illinois license.
• High school diploma or equivalent preferred.
• Active Illinois driver’s license required; Class C or E license as applicable.
• No driver under 25 or over 74; DOT physical required for drivers.
• Experience as a bus/van driver preferred; strong written and spoken communication.

Responsibilities

• Coordinate and implement restorative nursing care for patients under Nursing direction.
• Transport patients safely to hospitals, appointments, and residences.
• Maintain restorative program documentation and progress notes.
• Assist with transfer and mobility devices; train caregivers in restorative techniques.
• Monitor safety during transport and on facility grounds; ensure equipment is maintained.
• Communicate with nursing and rehabilitation staff about patient goals and progress.
• Complete daily vehicle inspections and logs; follow infection control procedures.
