Job summary

WESTERVILLE is seeking a qualified Social Service professional to plan, develop, organize, implement, and direct our facility's social service programs in accordance with federal, state and local standards.

The role requires collaboration with administration, medical and nursing staff, development of policies, and coordination with community agencies to meet residents' medically related social and emotional needs. The position emphasizes discharge planning, progress notes, and staff training.

Qualifications

• Must possess a high school diploma or equivalent; Associate or Bachelor's degree preferred but not required.
• Prior health-care experience preferred.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services.
• Assist in developing, administering and coordinating department policies and procedures.
• Review policies annually and participate in recommended changes.
• Coordinate social service activities with other departments as needed.
• Assist residents in achieving the highest practicable level of self-care and well-being.
• Provide information to residents/families about Medicare/Medicaid and other programs.
• Maintain regular social service progress notes and treatment plan responses.
• Assist in discharge planning and coordinating on-site ancillary services.
• Develop and participate in orientation and in-service training for social, emotional and medical needs.
