Job summary

Lakeland Health Care Center seeks a Social Services Manager to oversee planning, implementation, and evaluation of psychosocial services. You will lead the social services department, supervise staff, and advocate for resident rights while ensuring regulatory compliance.

You will collaborate with the interdisciplinary team, coordinate admissions, and guide discharge planning, with a focus on resident dignity and family support.

Qualifications

• Bachelor’s degree in social work; NASW membership standards; two years of social work experience in a health care setting.
• Master’s degree in social work from CSWE-accredited program; 6 months’ health care experience may substitute for two years.

Responsibilities

• Provide daily supervision, guidance, and consultation to program staff; review assignments and ensure compliance with rules.
• Conduct admission assessments and develop individualized social service care plans.
• Collaborate with the interdisciplinary team, residents, and families to plan safe discharges or locate alternative care settings.
• Develop and deliver staff training on residents’ rights and psychosocial needs of the elderly; contribute to policy and budget planning.
