Job summary

• Must possess, as a minimum, a high school diploma or its equivalent.
• Associate or Bachelor’s Degree preferred but not required.
• Prior health care experience preferred.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct social service programs with the Social Service Director.
• Meet with administration, medical and nursing staff to plan social services.
• Develop, administer and coordinate department policies and procedures.
• Identify medically related social and emotional needs of residents and assist in policy development.
• Coordinate social service activities with other departments and community agencies.
