Job summary

Marion Valley SNF Healthcare, LLC is seeking a Social Services/Administrative support professional to plan, implement and coordinate social services for residents. You will work with administration, medical staff, and families to ensure the highest possible well-being and coordinated care plans.

The role emphasizes policy development, discharge planning, and collaboration with community resources to support residents’ social and emotional needs.

Qualifications

• Must have a high school diploma or equivalent; Associate or Bachelor’s degree preferred but not required.
• Health-care experience preferred.
• Strong written and verbal communication and organizational skills.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services.
• Assist in developing and coordinating department policies and procedures.
• Review and suggest updates to department policies annually.
• Identify residents' medically related social and emotional needs and plan responses.
• Coordinate with community agencies to meet resident needs.
• Maintain progress notes on residents’ treatment plans and responses.
• Discharge planning with appropriate agencies and referrals.
• Develop and maintain rapport with other departments to support social services.
