Job summary

Responsibilities

• Assist with admission process: tours, inquiries, interviewing prospective residents or families.
• Arrange personal care items, transportation, and equipment as requested.
• Maintain contact with family and attending physicians for care plan meetings and communications.
• Document all contacts and referrals on the social service log.
• Assist residents and families with completing forms, including advance directives and Medi-Cal applications.
• Participate in Behavior Management programs and attend in-service meetings.
