Job summary

Laurel Health and Rehabilitation Center is seeking a Social Services & Activities Support professional to provide direct support to the Social Services Director while also assisting the Activities Department in promoting resident engagement, psychosocial well-being, and quality of life.

This dual-role position supports residents’ social, emotional, psychological, and recreational needs, contributing to individualized care planning throughout their stay and, when appropriate, during

Qualifications

• Associate's Degree and state certification required.
• 1–2 years of experience in geriatrics.
• Experience in Behavior Add-on Programs preferred.

Responsibilities

• Assist the Social Services Director with admission, quarterly, and discharge care conferences.
• Conduct and document Social Services Admission, Quarterly, and Comprehensive Evaluations.
• Assist with discharge planning, including coordination of services and resources.
• Arrange ancillary and community services as ordered or needed.
• Provide or arrange social service interventions based on resident needs.
• Review, document, and follow up on resident complaints and grievances per facility policy.
• Maintain resident confidentiality and accurate documentation within the electronic health record.
• Assist the Activities Department with planning, coordinating, and facilitating group and individual activities.
• Support activities that promote socialization, cognitive stimulation, emotional wellness, and resident choice.
• Encourage resident participation and engagement in activities and events.
• Assist with special events, outings, and community involvement activities.
