Job summary

Laurel Square Healthcare and Rehabilitation Center in Philadelphia is seeking an Activities Director to develop and implement a dynamic activity program that meets residents' needs and complies with federal and state requirements. You will work on planning individual and group activities, document plans and progress, and participate in resident care conferences with an interdisciplinary team.

The role also includes organizing Residents’ Council meetings and supervising volunteers.

Responsibilities

• Plan, schedule and implement activity programs for individuals and groups.
• Use RAPs to determine care planning needs related to activities.
• Document activity interests and care plan updates quarterly.
• Record interactions in assessments, care plans and progress notes per requirements.
• Participate in interdisciplinary team and attend care conferences.
• Plan and conduct Residents’ Council meetings.
• Recruit, schedule and supervise assistants and volunteers.
• Attend in-service sessions related to activity program.
• Organize community events aligned with residents’ interests.
• Plan in-room activities for residents unable to leave their rooms.
• Perform other duties as assigned by Administrator.

We are looking for an Activities Director to join our team.

The purpose of this position is to develop and implement an activity program in compliance with requirements to meet residents' needs.

Duties:

• Plan, schedule and implement a program of individual and group activities based on residents’ schedule.
• Use the resident assessment protocols (RAPs) to determine whether to proceed to care planning for concerns related to a resident’s activity program needs.
• Identify and document in the resident’s care plan, the activity interests, needs, and concerns of the resident. Care plans are reviewed and updated as necessary but at least quarterly.
• Document all interaction with resident and/or family in the assessment, care plan and progress notes as required by federal and state requirements.
• Participate as an active member of the interdisciplinary team and attend all resident care conferences.
• Plan and conduct Residents’ Council meetings.
• Recruit, schedule and supervise assistants and volunteers.
• Take part in in-service education programs and attend all in-service session relating to Activity Program.
• Organize and schedule community events related to residents’ interests.
• Plan, schedule and implement room visits and in-room activities for residents unable to leave their rooms.
• Other duties assigned by Administrator
