RHF's Vacaville community seeks a Social Service Coordinator to support residents by identifying needs and coordinating essential services, building relationships with providers, and advocating for residents. The role is full-time, 40 hours, at Vacaville Autumn Leaves, a 56-unit older adult residence in Vacaville, CA.

The position emphasizes service coordination, program development, crisis intervention, and collaboration with management and external agencies.

Qualifications

• Bachelor’s degree in Gerontology, Human Services, Social Work, Psychology, Sociology, or related field.
• Experience with diverse groups of seniors and disabled individuals preferred.

Responsibilities

• Assess residents' needs to identify services that enhance quality of life.
• Coordinate access to social, health, and wellness resources.
• Develop and sustain health and wellness programs for residents.
• Maintain directory of community resources and services.
• Advocate for residents and liaise with community services.
