• Establish and maintain connections with local service providers.
• Create and sustain educational health and wellness programs.
• Develop and maintain a directory of community resources for residents.
• Act as a resident advocate and liaise between residents and community services.
• Build and maintain relationships with community agencies.
• Foster effective communication between residents and service providers.
• Monitor resident referrals to external agencies and ensure service delivery.
• Maintain accurate records and documentation of service coordination.
