Aperion Care in Chicago seeks a Social Services Assistant to help plan, organize, implement, evaluate, and direct the Social Services Department in accordance with federal, state, and local standards, aiming to meet residents’ psychosocial needs.

You will collaborate with an interdisciplinary team to review policies, develop care plans, document progress, and advocate for residents and families, ensuring rights and quality of life are protected.

Qualificações

• Bachelor’s degree in a human services field.
• A minimum of one year of supervised experience in mental health or human services preferred.

Responsabilidades

• Review facility policies and procedures with the interdisciplinary team to ensure compliance with state and federal regulations.
• Participate in reviewing and setting policies concerning resident care and quality of life.
• Develop facility social work policies.
• Attend quality assurance interdisciplinary team meetings.
• Ensure government requirements for social service documentation are met.
• Document progress in addressing residents' psychosocial needs.
• Promote resident rights and psychological wellbeing, addressing abuse as required by law and licensure.
• Provide support to residents, families, and staff to facilitate proactive self-advocacy for better quality of life and care.
• Complete social histories and psychosocial assessments for residents.
