Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, or a related field is required.
• LSW or equivalent preferred; active licensure is valued.
• Experience in long-term care or skilled nursing facilities is preferred.
• Knowledge of CMS regulations, care planning, and discharge planning processes is important.

Responsibilities

• Oversees and manages the social services program in accordance with regulations.
• Conducts psychosocial assessments at admission and during stay.
• Develops and implements individualized care plans for emotional and social needs.
• Provides counseling and support to residents and families during transitions.
• Coordinates discharge planning for safe transitions to home or other care settings.
• Assists residents and families with accessing community resources.
• Participates in care plan meetings with the interdisciplinary team.
• Advocates for resident rights and helps resolve concerns or grievances.
• Maintains documentation to regulatory standards.
• Educates staff on psychosocial needs.
