Job summary

Northcrest Nursing & Rehabilitation Center in Napoleon, OH is seeking a compassionate Social Services Director to join our leadership team. This key role supports residents and families from admission through discharge, ensuring a positive and dignified experience.

You will lead the Social Services department, advocate for residents’ psychosocial needs, coordinate discharge planning, and collaborate with care teams to deliver person-centered care.

Qualifications

• Degree in Social Work or related field preferred.
• Previous social services experience in skilled nursing or long-term care preferred.
• Knowledge of resident rights and discharge planning a plus.
• Strong communication, organization, and problem-solving skills.
• Compassionate, professional, and team-oriented.

Responsibilities

• Lead and oversee the facility’s Social Services department.
• Advocate for residents and support psychosocial and emotional needs.
• Build strong relationships with residents, families, and responsible parties.
• Participate in care planning and interdisciplinary team meetings.
• Coordinate discharge planning and transitions of care.
• Connect residents and families with community resources and services.
• Address resident and family concerns promptly and professionally.
• Complete required assessments, documentation, and care plan updates.
• Support resident rights and regulatory compliance.
• Collaborate with Nursing, Admissions, Therapy, Activities, and leadership.
