Job summary

VOAMI in Michigan seeks a Service Coordinator to manage service provision for residents across assigned properties. The role involves liaison with providers, educating residents on resources, and monitoring services to ensure timely support.

You will collaborate with the Community Administrator and housing staff to help residents maintain independence, build informal support networks, and navigate leases while protecting confidentiality.

Qualifications

• Knowledge of community services in the region, and services for older adults.
• Understanding of aging processes, elder services, disability services, and mental health issues.
• Familiarity with eligibility for Federal and state entitlement programs.
• Ability to communicate confidential information and maintain records.

Responsibilities

• Develop contact with service providers and educate residents about resources.
• Monitor the delivery of services to ensure timeliness and appropriateness.
• Provide limited case management and develop service plans for residents.
• Educate residents on service availability, rights, and procedures.
• Advocate for residents and coordinate with community agencies.
