Pinewood Nursing Home is seeking a Social Services Designee to coordinate the continuum of care for residents. This role requires assessment, planning, implementation, coordination, monitoring, and evaluation of services to meet residents' needs.

Reports to the Licensed Social Worker for clinical review and documentation oversight. Responsibilities include admissions assessments, care plan meetings, post-discharge planning, and ensuring compliance with state and federal regulations while

Qualifications

• Bachelors or Associates degree in Nursing, Social Work, or a related healthcare field.
• Valid state licensure as RN, LPN, or LCSW (where required).
• Minimum 2 years of case management experience in long-term care preferred.
• Strong assessment and care planning skills.
• Excellent communication and interpersonal abilities.
• Proficiency with EHR and healthcare documentation.

Responsabilités

• Coordinate all aspects of resident care and medical treatments with the interdisciplinary team.
• Assess, plan, implement, monitor, and evaluate resident services.
• Lead care plan meetings with residents, families, and staff.
• Maintain accurate documentation in compliance with regulations and policies.
• Develop discharge plans and coordinate post-discharge services.
• Provide empathetic support to residents and families and address concerns promptly.
