Job summary

Water’s Edge Rehab & Nursing Center at Port Jefferson is seeking a compassionate Social Worker to join our interdisciplinary team in Port Jefferson, NY. The role focuses on emotional support, discharge planning, care coordination, and connecting residents and families with community resources to ensure the highest quality of care.

You will conduct psychosocial assessments, participate in care planning, advocate for resident rights, and collaborate with physicians, nursing staff, and social work

Qualifications

• Bachelor’s Degree in Social Work is required; Master’s Degree preferred.
• Experience in a skilled nursing facility or healthcare setting preferred.
• Strong communication, interpersonal, and organizational skills.
• Knowledge of discharge planning and long‑term care regulations preferred.
• Compassionate, dependable, and resident-focused.
• Ability to work effectively within an interdisciplinary team.

Responsibilities

• Provide emotional support and counseling to residents and families.
• Assist with admissions, discharge planning, and transition of care coordination.
• Conduct psychosocial assessments and maintain accurate documentation.
• Collaborate with residents, families, physicians, nursing staff, and interdisciplinary team members.
• Assist residents and families with accessing community resources and support services.
• Advocate for resident rights and promote resident-centered care.
• Participate in care plan meetings and interdisciplinary team discussions.
• Ensure compliance with all federal, state, and facility regulations and policies.
• Address resident and family concerns in a professional and timely manner.
• Maintain confidentiality and professionalism at all times.
