• Build and maintain a robust network of local agencies and wellness resources.
• Educate residents and families about federal, state, and local programs.

• Be the go-to liaison between residents and providers for timely support.
• Collaborate with management to identify residents in need and assist.
• Monitor service delivery to ensure high-quality care.
• Advocate for cost-effective, adequate services for residents.
• Create and maintain a directory of community resources.
